Below is a pdf form that you can fill up using your computer.
- Click the link to download the pdf file into your computer.
- It will prompt you to save the file onto a location in your computer. Remember this location and change the filename (SAVE AS) to have this format:
- SD-LASTNAMEFirstname of Primary Guardian/Parent (e.g. SD-DOEJohn.pdf)
- SFX-LASTNAMEFirstname of Primary Guardian/Parent (e.g. SFX-DOEJane.pdf)
- Fill up the form. When completed, don't forget to save again. This pdf file of the registration form is required to be attached for paymeny below.
- If you are exempt from paying the fees (e.g. you are a catechist) OR are applying for scholarship, SKIP THE PAYMENT and email the form to Angie: jacocatholicsre@gmail.com
